  1. Download Enrolment and Survey Form.

  1. Fill-out the Form.

  2. Submit the Form to the School with the following requirements:

a) Photocopy of Birth Certificate -PSA

b) School Record

c) 1x1 ID picture with white background

  1. Interview with the parents or guardians.

  2. Proceed to the Cashier for payment.
